I think this may fit the bill. It's called 'confusion corner' for this reason:

I figured Utica would be a good candidate here, and I quickly found a 5-shield sign with a bonus 6th on another sign on the same assembly.  April 28, 2016.

I've posted this one before, but here's a sign with five shields on I-95 northbound approaching I-395 in Bangor, ME. There would be a sixth shield if ME 15 was signed here.

US 202/US 322 on the West Chester, PA by-pass. These have been replaced.

Might as well get one of my hometown examples in here.  On NY 30 northbound over the Mohawk River in Amsterdam.  This one taken March 27, 2013.  As mentioned in another thread recently, these are scheduled for replacement soon.
