No German states?

I haven't been to Germany either. I've only been to 5 countries in my life, and none of them has states. Of course, I've crossed subnational borders lots of times, most notably every paved one between Aragon and Rioja (though it's not possible to reach the rest of Aragon from that area without crossing into either Castile and Leon or Navarre, or going on dirt tracks).

There are way too many to list, but I will say I have crossed every single PA-MD crossing where the PA side is a State/US/Interstate Route.

VT/MA: I-91 and US 7

MA/RI: I-95, I-295, I-195, state 146...Attleboro into Pawtucket via US 1 on foot, MBTA commuter rail at least three times

PA/DE: I-95 and Amtrak

PA/MD: June 2014...just know it was heading NW out of Elkton to eventually Harrisburg (by car)

DE/MD: I-95, Amtrak and state 273

MD/DC: Baltimore/Washington Parkway, Amtrak and METRO (red line to/from Silver Spring)

DC/VA: METRO and Key Bridge (on foot from Rosslyn part of Arlington into Georgetown)

IL/IN: NICTD commuter train from Millennium Park Chicago into Hammond (was in Indiana by maybe 4 blocks?)

I've also been in NC, FL, TN and OH. Those were all with flight, with no state lines crossed. Chicago was by flight to and from Bradley Airport in Windsor Locks, CT before and after local crossing.

NOTE: I've never been in Canada or Mexico.

As a passenger in a car, I can't name all the crossings, but via bus and train I can add the following (not including duplicates of the above):
DC/MD: US 50 (I think)
MD/DE: AMTRAK Northeast Corridor
DE/PA: AMTRAK Northeast Corridor
PA/NJ: PATCO, AMTRAK Northeast Corridor / SEPTA Trenton line
NJ/NY: NJ/NY 495, PATH (both tunnels), NJ Transit Northeast Corridor
NY/CT: Metro North New Haven Line, I-84 (I think)
CT/MA: I-84 (I think)

Curiously, when going somewhere by plane, I haven't ever crossed any state borders once on the ground (which means I've been to far more states than the above suggests)

Living in Michigan and working in Toledo, OH, I've crossed probably all border crossings between Lake Erie and Indiana (including Lost Peninsula).  For Michigan-Indiana, the main ones - IN-120, I-69, I-94.  For Michigan-Wisconsin, US-2, US-8, US 41.  For OH-WV, I-77, US-50, both bridges in between them, US-35, and another local road.

Since your guys have started posting Canadian provinces (which technically are not states), might as well as do European subdivisions. Note that all  crossings involving a French region apart from all Aragon/New Aquitanie crossings were done before the new regions came into existence, list shows current regions.
