Usually:
Mayonnaise, but sometimes I'll switch it up for 1000 Island or Tartar Sauce (one local chain here serves their burgers with Tartar Sauce)
Onion - usually raw, but sometimes grilled
Tomato - if not available, a little catsup.  Tomatoes and catsup are an either/or thing for me.   I never put both on the same burger.
Dill Pickles
Lettuce
Cheese - Cheddar, Gouda, or Swiss, but I'll eat any cheese that's offered (even the "dreaded American")
When I make burgers at home, this is how I fix them.

Sometimes (depending on my mood, and also how much extra they are charging):
Mushroom - only if they're grilled/sauteed
Bacon
Fried Onions/Onion Straws

Rarely/Never:
Mustard
Jalapenos
BBQ Sauce
Fried Egg
Cole Slaw
Chili
(some of these are things I really like, just not on a burger)

When I make them at home, I add the following to the ground beef before grilling:  Montreal Steak Seasoning, finely diced onion, shredded cheddar, and a little mayonnaise (instead of egg as some do)

I immediately thought of this, but that works as well.

Bacon might be more common, but ham on a burger is far from rare.

My burger of choice comes from Patrick's Butcher Boy

Either grilled onion and ketchup  OR bacon and BBQ sauce.

I like switching around, though it confuses the counter help.
