U.S. 19 travels 89.03 miles through southwestern reaches of the Commonwealth of Virginia, connecting Bristol, Tennessee with Lebanon, Tazewell, and Bluefield at the West Virginia state line. North of Abingdon, U.S. 19 maintains four lanes with limited access bypasses of Lebanon and Tazewell.

Entering the state with U.S. 11E-421 from Bristol, Tennessee, U.S. 19 follows Commonwealth Avenue through the independent city of Bristol 0.60 miles north to Lee Highway. While U.S. 421 turns westerly toward Gate City, U.S. 11-19 angles northeast to Abingdon in neighboring Washington County, running close to I-81. U.S. 19 joins U.S. 58 Alternate on the western side of Abingdon, seat of Washington County, as U.S. 11 continues northeast to Wytheville and Fort Chiswell. Following Porterfield Highway northwest, U.S. 19 and U.S. 58 Alternate pass through the Clinch Mountain range.

U.S. 19 and U.S. 58 Alternate part ways at the community of Hansonville, with U.S. 19 turning back northeast toward Lebanon, seat of Russell County. U.S. 19 lines the south side of Lebanon along a limited access expressway while a business route follows the old alignment through downtown. Continuing northeast along the Clinch Mountain range, U.S. 19 passes through Rosedale and Wardell en route to Claypool Hill and U.S. 460. U.S. 19-460 overlay the Governor George C. Peery Highway, paralleling the Clinch River toward Tazewell.

U.S. 19-460 Business leave for Tazewell at Pisgah while U.S. 19-460 stay to the north along a second limited access expressway. Leaving the Tazewell County seat, U.S. 19-460 advances northeast between East River Mountain and Little Valley Ridge to the outskirts of Bluefield, where the two highways diverge. U.S. 19 leaves U.S. 460 at the western reaches of Bluefield for downtown while U.S. 460 follows a limited access bypass to the south. Within the town of Bluefield, U.S. 19 travels along Virginia Avenue. The U.S. highway enters the city of Bluefield at the West Virginia state line.
