One of the shorter routes in the U.S. Highway system, U.S. 123 measures just 77.9 miles overall from U.S. 23 outside Toccoa, Georgia to Business Spur I-385 in Greenville, South Carolina. U.S. 123 enters the Palmetto State across Lake Hartwell along a two lane alignment to the city of Westminster. There the route combines with U.S. 76 through the city center and eventually expands into a four lane, divided highway running east to the outskirts of Seneca.

U.S. 76/123 transition into a arterial along the north side of Seneca. The alignment bypasses Downtown and the associated street grid while remaining commercial. Advancing east, the pair approach the city of Clemson and cross the Seneca River arm of Lake Hartwell. U.S. 123 again lines a commercial arterial to the split with U.S. 76, which drops south to Anderson. East from there, U.S. 123 upgrades to a freeway bypassing Central, Liberty and the west side of Easley.

Beyond the interchange with S.C. 135 (Pendleton Street), U.S. 123 lines a busy commercial route through the rest of Easley to the junction with S.C. 153. S.C. 153 leads motorists south to I-85 to bypass Greenville, while U.S. 123 continues toward the Upstate's largest city.

The final stretch of U.S. 123 takes the highway east along Easley Bridge Road to an alignment encircling Downtown Greenville along Academy Street. U.S. 123 concludes at North Street (unsigned Business Spur I-385) at Williams Street, just east of U.S. 29 and west of Interstate 385.

Historically U.S. 123 turned east on Pendleton Street from Easley Bridge Road in Greenville to join Main Street north into Downtown. S.C. 124 west along Pendleton Street and Old Easley Highway was designated then as U.S. 123 Alternate.
