U.S. 222 extends south 3.61 miles into Maryland from Lancaster County, Pennsylvania. Following Rock Springs Road, the highway connects with U.S. 1 (Conowingo Road) and MD 222 at Conowingo.
Prior to 1995, U.S. 222 continued southward along an overlap with U.S. 1 to Conowingo Dam and alongside the Susquehanna River to I-95, U.S. 40 and MD 7 at Perryville. This alignment included a narrow stretch along Main Street through Port Deposit that precluded most truck traffic. A Truck route for U.S. 222 was established to the east along the combination of U.S. 1 north, MD 276 south and MD 275 south.
AASHTO formally approved the elimination of U.S. 222 south of U.S. 1 on April 23, 1995. U.S. 222 was subsequently renumbered as a state route and the overlap along U.S. 1 dropped.
