SR 21 begins at the state line, extending northward from Florida State Road 97 to Atmore in Escambia County. SR 21 and SR 97 provide an alternate route to Pensacola, Florida in lieu of U.S. 29 and Alabama State Route 113 to the east.

SR 21 constitutes a four lane arterial southward from Interstate 65 into the city of Atmore. North beyond I-65, SR 21 connects the rural towns of Uriah and Frisco City with Monroeville along a two lane highway. Within Frisco City, a signed truck route for SR 21 bypasses the main portion of town to the east. Within the vicinity of Monroeville, SR 21 crosses U.S. 84 and combines with SR 41 northward on a four lane alignment. Upon reaching central Monroeville, SR 21 splits between a western Business alignment and an eastern four lane Bypass alignment. The separation is two miles, with SR 47 tying into SR 21 Business at Downtown Monroeville.

Further north, SR 21 overlaps with U.S. 80 leading into Montgomery and with U.S. 80, U.S. 82 and U.S. 231 around Montgomery on the South and East Boulevard system. U.S. 231 and SR 21 combine through Wetumpka and Sylacauga, where the state route shifts to a separate alignment northeast to Talladega and Anniston.
