County Road 545 follows a linear route along Old Lake Wilson Road north from County Road 532 (Osceola Polk Line Road) to U.S. 192 in northwestern Osceola County. The 5.180-mile route includes a 0.295 mile long segment on Morgan Williams Road, which was bypassed by the extension of the four-lane Old Lake Wilson Road north to U.S. 192. Suburban development overtook the majority of the timberland and fruit tree groves along CR 545, with increasing traffic resulting in future widening of Lake Wilson Road south of CR 532 to Ronald Reagan Parkway (CR 54) in Polk County. Areas north of the directional T interchange joining I-4 with SR 429 (Western Beltway) were developed in the mid 2010's.

A second branch of County Road 545 extends north from U.S. 192 along Avalon Road in western Orange County. Avalon Road leads CR 545 through the Horizon West area and Winter Garden.
