County Road F12 begins at County Road F15 north of Marengo. The route follows MM Avenue north to 110th Street east across northern Iowa County to Monroe township in Johnson County. Following 120th Street, CR F12 reaches the city of Swisher as Division Street. Beyond the city line, CR F12 resumes to cross Interstate 380, U.S. 218/Iowa 27 on the west side of Shueyville.
East from Shueyville, Johnson County F12 lines 120th Street to Ely Road (CR W6E) south. CR W6E branches south along Ely Road to Solon while CR F12 rejoins 120th Street east to end at Sutliff Road (CR F14) in Cedar township.
