SR 204 Spur was a 7.18 mile long route joining the city of Savannah at White Bluff to Montgomery, Skidaway Island and Skidaway Island State Park in coastal Chatham County. Established by 1990, the state route followed a commercialized stretch of Montgomery Cross Road east from SR 204 (Abercorn Street) to Waters Avenue. SR 204SP turned south on Waters Avenue across Haneys Creek to the Halcyon Bluff neighborhood.
Whitefield Avenue overtakes Waters Avenue south leading to a diamond interchange with Truman Parkway. Whitefield Avenue branches southwest to Montgomery while SR 204SP transitioned onto Diamond Causeway ahead of the Pin Point community. Diamond Causeway crosses the Moon River south of the Wormsloe Historic Site and the Skidaway River by the Rodney J. Hall Boat Ramp. SR 204SP concluded at the four way stop with Green Island Road south, McWhorter Drive north and Tidewater Way into The Landings gated community.
Construction completed in December 2013 replaced the draw bridge along Diamond Causeway spanning Skidaway Narrows. A high rise span with a 65 foot clearance replaced the previous span built in 1970. A mileage transfer between the Georgia Department of Transportation (GDOT) and Chatham County in 2020 shifted state maintenance from all of SR 204SP and SR 204 north of SR 21 (DeRenne Avenue) in exchange for Jimmy DeLoach Parkway between Bloomingdale and SR 21 ALT at Port Wentworth. Jimmy DeLoach Parkway was designated as an eastward extension of SR 17.

Truman Parkway opened north from Whitefield Avenue to Montgomery Crossroad on September 22, 2005. Work on the freeway west to SR 204 commenced in March 2010.
