Forming a multi state highway with Missouri Route 5, Iowa 5 extends 104 miles north from Cincinnati near the state line to Interstate 35 in West Des Moines. The route travels north through Centerville, Albia and Knoxville, where it combines with Iowa 92 along an expressway. Iowa 5 splits with MSR 92 near Pleasantville and continues northwest to Carlisle and U.S. 65 at Des Moines. Within the Des Moines metro area, Iowa 5 forms the southern leg of an outer beltway connecting I-35 at West Des Moines with I-80 at Altoona.

The Des Moines outer beltway straddles the Polk and Warren County line east from Interstate 35 in West Des Moines to Norwalk and southeastern reaches of Des Moines. U.S. 65 combines with Iowa 5 east from U.S. 69 to the succeeding interchange where Iowa 5 branches southeast along an expressway through Carlisle. U.S. 65 travels along the remainder of the beltway north to the city of Pleasant Hill, and between the northeastern Des Moines city line and Altoona, to Interstate 80. The Iowa 5 section of the freeway loop opened initially between Iowa 28 and U.S. 65/69 in January 2001 and fully by October 2002.1
