Illinois Route 203 travels just 9.7 miles along an arterial linking Madison and Granite City with I-55/70 to the south with Interstate 255 to the north. The state route follows a portion of old U.S. 67 Alternate, which utilized Madison Road north from Collinsville Road (old U.S. 40 & 66) north to Horseshoe Lake. The four-lane divided highway portion of Route 203 replaced Madison Road to the west.

Northward into Madison, Route 203 and former U.S. 67 Alternate follow McCambridge Avenue to Edwardsville Road northeast. U.S. 67 Alternate used 20th Street northwest to Missouri Avenue through central Granite City, while Route 203 continues Edwardsville Road northeast to Nameoki Road north at Route 162.

Illinois Route 203 North
Collinsville Road (old U.S. 40 & 66) eastbound at the northbound beginning of Route 203. The original split of U.S. 67 Alternate north from U.S. 40 & 66 east was located just to the west of this intersection. That alignment was abandoned when the adjacent interchange with I-55/70 was built. 12/20/09
Route 203 commences northward toward Madison along a four lane divided highway. A directional cloverleaf interchange immediately joins the state route with I-55/70 & U.S. 40. The Great River Road accompanies the freeway east to Route 111 north and west to Route 3 south. 05/24/18

A loop ramp follows from Route 203 north onto I-55 south/U.S. 40 west to the Poplar Street Bridge and I-70 west to the Stan Musual Veterans Memorial Bridge into St. Louis, Missouri. 05/24/18
Illinois Route 203 South
Illinois Route 203 south ends at Collinsville Road (former U.S. 40 & 66). Collinsville Road leads southwest as 9th Street in East St. Louis and east to Fairmont City. 05/24/18, 12/20/09
