Measuring 79.79 miles, NC 37 leads north from U.S. 64 in Washington County to U.S. 13 in Gates County within northeastern North Carolina. NC 37 crosses the Albemarle Sound Bridge alongside NC 32 and NC 94 between Leonards Point and Sandy Point south of St. Johns. NC 37 overlaps with U.S. 17/Future I-87 northeast to the Perquimans County seat of Hertford before turning northwest from Winfall along the Perquimans River to Belvidere.

NC 37 combines with NC 32 for a second time north from Chowan County to Mintonsville and Mitchells Fork in southern Gates County. NC 37 turns west from NC 32 to Gatesville, where it joins U.S. 158 Business northward. The state route concludes north of Gates in an agricultural area near Suffolk, Virginia.
