North Carolina Highway 45 travels northwest from the ferry landing joining Swan Quarter with Ocracoke in Hyde County to the Hertford County seat of Winton. The 96.94 mile long state route joins the Hyde County seat of Swan Quarter with Plymouth in western Washington County. This includes a 15.66 mile long overlap with U.S. 264.
NC 45 combines with NC 308 across the Roanoke River north from U.S. 64 to Bertie County. Paralleling the Chowan River to the west, NC 45 continues to Colerain, Harrellsville and Cofield. The north end ties into the west split of U.S. 13 and U.S. 158 in the town of Winton.
