U.S. 41 spans a total of 104.94 miles through Kentucky between Guthrie at the Tennessee state line and the Ohio River south of Evansville, Indiana. The U.S. highway typically runs parallel to I-24, I-69, and future I-169 along its course through the Bluegrass State.

Entering the state at Guthrie in Todd County, U.S. 41 follows both State and Park Streets to U.S. 79 in the west part of the city. Turning northwest along Dixie Beeline Highway, U.S. 41 advances to Trenton before crossing into Christian County ahead of Pembroke. Entering Hopkinsville from the southeast along Pembroke Road, U.S. 41 meets KY 109 ahead of a diamond interchange with the Edward T. Breathitt (Pennyrile) Parkway (future Interstate 169). E 9th Street continues U.S. 41/KY 109 northwest to a merge with U.S. 68/KY 80 west ahead of S Walnut Street (U.S. 41 Alternate) and downtown. One way pairs (S Main and Virginia Streets) lead U.S. 41 away from downtown to northern reaches of Hopkinsville and KY 1682 (Eagle Way).
Beyond Hopkinsville, U.S. 41 parallels future I-169 (Edward T. Breathitt Parkway) on a rural course north to Crofton and into Hopkins County. A direct ramp joins U.S. 41 with northbound Pennyrile Parkway ahead of U.S. 62 and Nortonville. Turning northwest through Little Valley, U.S. 41 passes under Interstate 69 (Wendell H. Ford Parkway) en route to Mortons Gap. I-69 joins the Pennyrile Parkway as U.S. 41 winds northerly between Mortons Gap and Earlington into Madisonville, seat of Hopkins County. U.S. 41 lines S Main Street into downtown, with KY 70 joining for a 0.70 mile overlap between W McLaughlin Avenue and E Center Street. Continuing along N Main Street, U.S. 41 advances to a second branch of U.S. 41 Alternate in the state. U.S. 41 ALT travels west to Providence before turning north for Dixon and Henderson while U.S. 41 provides a more linear route to the Henderson County seat.
Leaving Madisonville, U.S. 41 proceeds north along side I-69 (Pennyrile Parkway) through Hanson before shifting westward to the CSX siding of Slaughters. Advancing northwest through Webster County, U.S. 41 continues to Sebree. Converging on a parallel course with I-69 once more, U.S. 41 enters Henderson County ahead of Robards. Meeting KY 425 (Henderson Bypass), U.S. 41 turns east to join the Pennyrile Parkway south of the western end of the Audubon Parkway (KY 9005). U.S. 41 overlays the Pennyrile Parkway to its conclusion at a directional cloverleaf interchange with U.S. 60/41 ALT north of downtown. The remainder of U.S. 41 through Henderson is a commercially lined arterial to the edge of John James Audubon State Park. U.S. 41 concludes its run through the Bluegrass State crossing the Ohio River along twin steel truss cantilever spans. Built in 1932 (southbound) and 1965 (northbound), the bridges carry U.S. 41 onto Green River Island ahead of the Indiana state line and Evanville.
