U.S. 221 passes through western North Carolina from Chesnee, South Carolina, south of Forest City to Independence, Virginia, north of Twin Oaks. U.S. 221 joins Spindale and Rutherfordton with the McDowell County seat of Marion. A four lane section leads U.S. 221 north from Lake James into a valley between Woods Mountain and Bald Mountain.

Paralleling the North Fork of the Catawba River along Linville Mountain, U.S. 221 advances north through Pisgah National Forest to Linville Falls in the Blue Ridge Mountains. U.S. 221 heads east by Grandfather Mountain to Blowing Rock, where it combines with U.S. 321 north to U.S. 421 at Boone. U.S. 221/421 overlap east to Deep Gap in the Blue Ridge Mountains. U.S. 221 resumes a northward heading from Deep Gap into Ashe County. U.S. 221 combines with U.S. 21 north from Twin Oaks to exit the state.
