Beginning in Downtown Cleveland, U.S. 422 in Ohio runs 74.96 miles southeast to Warren, Youngstown and the Pennsylvania state line. U.S. 422 overlaps with SR 43/87 east from Public Square along Ontario Street to I-90 (Innerbelt Freeway) at Orange Avenue. U.S. 422/SR 87 continue onto Woodland Avenue to E 55th Street and Kinsman Road in Cleveland's Central Neighborhood.

U.S. 422 shifts southeast onto Kinsman Road alongside SR 8 from SR 87 (Woodland Avenue) to Kinsman, Woodland Hills and Mt. Pleasant in Cleveland. The two routes part ways in the city of Shaker Heights where Chagrin Boulevard intersects Warrensville Center Road. U.S. 422 remains along Chagrin Boulevard east to Interstate 271.

Combining with I-271, U.S. 422 turns south to I-480N in Warrensville Heights and a freeway spur extending east to Solon and Auburn Corners in Geagua County. A short two lane section of U.S. 422 leads east to Welshfield. U.S. 422 expands to four lanes southeast from Parkman to the city of Warren.

U.S. 422 constitutes a commercial boulevard southeast from Warren into the cities of Niles and Girard. Within Youngstown, U.S. 422 bypasses Downtown and Youngstown State University along the Madison Avenue Freeway to U.S. 62 at Oak Street.
