(So do I play the L from Hospital or the H from South?)

"Hospital South" is the name. There are also nearby Hospital North and Hospital West, all different streets.

Which, IMO, would make "South", "North", and "West" all directionals, and so disregarded. The argument would work better if there were no Hospital North or Hospital West, and especially if Hospital South were not in the South half of town.

But too late to unwind this.

I've generally assumed the directional rule to mean things like prefixes/suffixes in a grid system, or other instances where, say, W Main St turns into E Main St but is clearly still the same street. I don't think this quite fits that.

Either way, no big deal. It was an H_____H, so if you wish just pretend I didn't play at all there.

If I read the rules correctly, "old" doesn't count... unless the rules were changed again.

Which rule? The first post in this thread doesn't have any such rule, and the OP has been pretty good about keeping that list up to date. Many Old ___ names have been played, and always in response to a name ending in "o".

"Old Highway 51" plays as F(ifty-on)E

Does this not apply to other road names, as well?

I think that just means "Old Highway" is treated as a road type, like "Highway". But not that "Old" can/must be generally ignored. There is a separate rule listing parts of names (like "Dr.", "Jr.", or "Memorial") that can be treated as optional, but "Old" is not on that list.
