When you pass by almost any major intersection in Hopewell, VA, they almost ALWAYS have mistakes

VA-10, VA-36, once I-295 (I don't quite remember but I think it was a painted US Shield), VA-156 ALL HAVE US SHIELDS. Just drive through the town, you'll find one!

I haven't seen any US 295 shields in Hopewell, where at?

I have never seen one either.

Although typical for temporary signage, still a mix up.
Every day is a winding road, you just got to get used to it.

Several of these sign goofs exist along US 522 south of McConnellsburg, PA.

Interstate 10 gets a mini downgrade.  I don't call it a full downgrade because there is a shield next to it showing it's true identify.

This brings up a philosophical question:  In California, since every highway is defined legally by the state as "Route XX" regardless of designation (thus meaning every highway in the state is technically a state highway), is it really an error when a US or Interstate highway gets the state highway treatment?

Let me quote someone here: Still Route 10 no matter how you slice it™.

I thought Californians called it "The 10".

That one's been there for quite some time.

How many instances have there been where a US / State route got upgraded to a blue Interstate shield?

I thought I saw photos of Interstate shields used on construction signage which mistakenly used for US and State routes/highways, but have not actually seen one in the field.

California has one each for US50, CA 99, and US 101 in the Shield Gallery. I thought someone posted an I-152 shield somewhere, but maybe I just imagined it.

I think I've seen those shield errors you're referring to, including the I-152 error, which I believe was on an exit sign off of I-5 near Los Banos?

Interstate 52, anyone? Picture taken today in La Jolla, CA.

That's obviously wrong; it's hundreds of miles away from Interstate 152:

(However, Caltrans fixed that sign some time ago.)

Texas had an I-90 shield up near San Antonio once.

Both PA and NY are notorious for mixups. :bigass:

In Middletown, NY, there is this overhead BGS on N Galleria Dr approaching NY-211. Here, NY-17 is upgraded to an eastern version of I-17 (they jumped the gun on the interstate designation and got the number wrong), and two new suffixed US Routes are added: US-211E and US-211W.
That is the only one that I can remember seeing in person off the top of my head, though I'm sure it's not the only one in the area.

There was also U.S. 29 signed as Interstate 29 in Washington:

Oscar Voss' photo from 12/09/00, posted on his A few Washington, D.C. area sign goofs web page.

There was also a post on Facebook a year or two ago showing an Interstate 84 shield posted for Arizona State Route 84.

Do business loops count?  Should be Texas State Highway Loop 363.

US 222 south in Maryland just past the Pennsylvania line marked as MD 222. This was a US 222 shield every previous time I drove past, and as late as 2021 per GSV.
From the land of red, white, yellow and black.

Is the erroneous US 34 shield in Gettysburg, PA still standing?
