AFAIK, there is only one place in Oregon where a 4 in the tens place meets a 1 in the tens place, and that's the junction of OR 141 and OR 210 in Beaverton. 

However, OR 141 is completely unsigned, and probably never will be since the sections of it ODOT maintains are now discontinuous.  So while I have a photo of the intersection where OR 141 begins, there is no 141 shield to be seen.

In the photo, OR 141 begins taking Hall Blvd to the left. Going straight is OR 210, which leads to an interchange with OR 217 in a couple blocks.

So this is OR 210 and OR 141:



OR 10 and OR 43 come close enough together that there are some assemblies carrying both shields:

The only example I know of in Oregon is the western terminus of OR 153 at OR 18 in Bellevue. Here's what that looks actually looks like, from the POV of westbound 153:



Unfortunately, OR 153 is not signed at this junction, or anywhere near it.  In fact, the route is barely signed at all, with just two shields posted way over by its eastern terminus:
