For Colorado -- Without looking at any reference material:
Interstate: I-270
US Highway: Used to be US-666, but I believe it's "replacement", US-491, still keeps it at the highest (??)
"Private" (Tolled) Highway: E-470 -- even has it's own styled shield.
State Highway: Used to be CO-789 (although still listed on some mappery), I want to guess C(O)-470 (??)
Massachusetts: I-495, MA 295, US 202
OHIO (again, from memory):
Interstate: I-490
US: US-422
State: OH-821 (??)
Alaska: AK 98. There are higher-numbered Forest Service roads, but I don't know their signage status.
Hawaii: Interstate H-201, HI 8930. For the county routes probably CR 581 on Kauai, with perhaps one remnant CR 803 sign on one of Oahu's no-longer-officially-numbered county routes.
Going into Canada's Arctic territories:
Yukon: YT 37
Northwest Territories: NT 8
Nunavut: no highways there
Also, Quebec's Autoroutes top out at A-973. I don't know about its other provincial routes, which go at least into the high 300s.

AAA jumped the gun and marked it as I-576 on a Pittsburgh city map some years ago, but it is most definitely not an Interstate yet. It's PA Turnpike 576.

CA-905 is the highest signed state route, although it will eventually be an interstate.

For your bonus, see F-97. As for signed county roads in general, the older system in the state used 3-digit numbers in a repeating grid of sorts. Here in Marquette County, we're still using that scheme, and we run up to CR 611. Leelanau County also uses that older system, and they're signed in the 600s as well, with at least a CR 643.

Is there a provincial trunk (primary system) highway in Manitoba numbered higher than 101? I know the PTHs don't go into the 200s.
190 is the highest PTH route I know of in the province. It was opened about 1-2 years ago and is in the works for being extended west to Headingley. Here is a picture:

Saskatchewan's northernmost secondary routes go up into the 900s, though I don't know the highest off the top of my head. At least one of its 900s is legendarily awful.
According to Wikipedia, highest is 999, but once again, I don't think it's signed.
