Exit Numbers and Route Numbers

A close call on I-295 SB in NJ: Exit 30 is for Warwick Rd, Exit 29 is for US 30.

Close but no cigar.  A similar scenario exists for MA 24 in Fall River, Exit 5 being for US 6.

That said & IMHO, neither one of the above-two examples would count.

Exit 75 for I-64 (eastbound only) in Kentucky is for I-75.

Exit 25 on I-75 is for US 25W.

If West Virginia signed its Exit 34 on I-64 the way it does most of its exits, that would be for WV 34. However, it's signed for CR 19. Contrast that with the previous exit for Milton, which is signed for US 60 but is actually a county route as well.

US 41, Exit 41, I-465 Indy
Bunch of others.  Too lazy to think.

I-465 does not have an Exit 41, and anyway US 41 does not come within 80 miles of I-465.

I suspect either a typo or a brain hiccup.

Exit 13 on I-664 is for US 13 (along with 58 and 460).

Exit 127 (A & B) on US 127 in Michigan is for both US 127 Business Loops.  B is for Alma's business route and A is for St. Louis' business route.

If NJDOT did have exit numbers for NJ 495 you might have its US 1 exit as Exit 1 (or 1B & 1C WB as 1A would have to be NJ 3).

In Florida you have Exit 33 on I-4 EB that connects with FL 33.

One that is posted in New Jersey . . . I-287, Exit 1 is U.S. 1.
