Has a toll ever been added to an existing road?

One very (& thankfully) short-lived example of such was the replacement Sakonnet River Bridge (RI 24/138) linking Portsmouth & Tiverton.  The old bridge, which was to be demolished, had no toll.  A plan make the new bridge AET nearly became reality but a revolt (regarding the high toll rates, especially for out-of-state motorists & truckers) caused that plan to be shelved though an interim 10-cent toll was charged for a short period.

I-95 in NC is going through a $4.4B rebuild/expansion in which the FHA has agreed the state can toll all road users.  The construction should be starting later this year.
Didn't I-70 in MO get approval from the FHA for something similar in the recent decade?  (having tolls added after reconstruction was finalized)

The I-95 toll has virtually no chance of happening. It's approved, sure but there was a big blow up a few years ago where the locals were ardently against it ever happening. The I-95 tolls are political poison in eastern North Carolina.

Well NC got one step further down than road than we did up here in PA.  That I-80 tolling fiasco still blows my mind.
