Posting cloverleaf interchanges that have four consecutive overpasses.  Typically 2 mainline interchanges with 2 C/D roadways.

I-269 at I-22 in northwest Mississippi is under construction.

I-295 at I-64 east near Richmond is another example, with an extra flyover for entertainment value.

Dallas North Tollway at I-635 appears to have five.  It has the mainlanes and C/D lanes as well as managed lanes.  In Street View, it looks like the managed lanes are on one structure.  The westbound C/D lanes are definitely on a separate structure, because the beams are at a different elevation.  The eastbound C/D lanes appear to have their own structure, as shown in the linked view, but I'm not certain.

Two in Michigan:

I-94 at I-69 northwest of Marshall

I-69 at US-127 east of Dewitt -- this one cheats a little since the southbound US-127 C/D lane ends before the ramp from eastbound I-69 merges in.  That ramp, by the way, used to be the mainline before the freeways were extended to the north and east.  Also note how all four loop ramps appear to have different radii.

Technically that's an example with ramps only via frontage roads and not C/D roads. Probably a few more of the type are lurking around in Texas.

The defining characteristic for this thread is four separate overpasses.  The OP said "typically" that will happen with C/D roads as an example, but frontage roads (or express/local lanes or anything else that might cause this) definitely qualify.

Yes there are. I drive through there every day. The C/D lanes and through lanes have almost no gap between but are separate structures.
