Double left turn lanes without protected-only phasing

How many occurrences are there where there is a double left turn lane (or a left + shared thru-left) on a two way street onto either a one way or another two way street that are not "left on arrow only" (something many manuals say should not be done)?

Kind of hard to see it (might have to spin the view around a bit), but at the intersection of Phillips and N. Detroit Avenues there is an unprotected left (2 left turn lanes) from westbound Phillips to southbound Detroit.

SB Pueblo Blvd. (CO-45) at Spaulding Avenue in Pueblo. This signal uses two four-section heads with FYA, but the signal never operates in protected-only mode. Since it's currently a T-intersection (future extension of Spaulding west is possible), there is no oncoming protected left phase that would justify the use of FYA instead of the five-section doghouses formerly used here.

Pass Rd at MS 605/Cowan-Lorraine Rd in Gulfport, MS.  All four legs have dual left-turn lanes.  The MS 605 lanes are protected-only while the Pass Rd lanes are protected-permitted.


Meanwhile, we have the curious case of VA 155 at SR 1110/Kentland Trl, just south of I-64 in New Kent County.  Southbound VA 155 has a dual-left but there are no traffic signals here.  Only control here is Kentland Trl has a stop sign.  I'm guessing the dual lefts here are to facilitate traffic coming off I-64 headed to the Colonial Downs racetrack down Kentland Trl....they certainly aren't warranted during "normal traffic".

I do not see this set-up often times; however, I am familiar with a couple that exist in various locations of New York City.
Some that I remember were altered over the years, such as one that I remember from the borough of Staten Island.

The set-up had one left turn (only) lane, while the lane next to it was both a thru and left turn lane. Below, is a street view of this intersection on Staten Island.

It was not until a couple of years ago that this set-up was altered. Aside from the new signals that were installed, the original two lanes were somewhat changed. The second lane is now solely a left turn lane.
