Interchanges where 2 lane has ramps to a multilane road

Above I have a link to the Wis 13 and US 8 interchange.  Wis 13 is 2 lanes while US 8 is 4 lanes.  The interchange gave priority to Wis 13 with only 2 lanes despite making US 8 four lanes in the interchange area.  What are some other interchanges that have this situation?

National Parkways such as the Natchez Trace Parkway do not count since they're mostly Super 2's built by the National Park Service.

TN 100 at TN 18 sort-of fits; TN 18 is planned to be widened from US 45 south to US 64.

I think most of the examples you're going to find are Super 2's though.

US 220 at US 158 in NC was like this but that's being reconstructed.

Have one from a 2 lane road to a 3 lane road where CT 10 meets US 6 in Farmington.  There is only a single ramp from CT 10 north to US 6 east, but CT 10 is a 2 lane road while US 6 is 3 lanes (2 east, 1 west) where they meet. 

And there are countless numbers of these in NJ.

If I misunderstood, the OP wanted an interchange where the multilane road allows a left turn onto a ramp entering a two-lane road. I don't know anything like that in NJ. If it's just a full interchange with no median breaks then yes, plenty of those in NJ.

Essentially the two-lane road has to be the freeway through the interchange, but cannot be a freeway on ether side.

The Santa Barbara example doesn't count only because SR 154 is officially a freeway (and signed as such).

US-30 and IL-31 in Montgomery, IL.  US-30 is a two-lane west of the interchange, and until recently, was a two-lane east of the interchange with a four-lane bridge over the Fox River.  IL-31 is six-lanes through the interchange.  Should US-30 be upgraded further, it will be a six-lane yielding to a four-lane.

There were a lot of super 2's in North Carolina at one point  (mostly town by-passes) but I don't think there's hardly any left today. That interchange you mentioned might very well be the last one. I could be wrong though

In Ontario, a 2-lane Hwy 3 (staged freeway) at 4-lane First Ave, St. Thomas, and the 3-lane Hwy 34 at CR-174 (formerly Hwy 17) at Hawkesbury.

Isn't that the opposite of what the OP was looking for, as 28 is 4 lanes where as 125 is 2, and 125 involves the stops and turns to get onto the ramps as opposed to 28.

What about state parkways?  If they count, then the Bethpage Parkway on Long Island is a super-2 and has an interchange with the four-lane Hempstead Turnpike (NY 24), which would probably match what you're describing.
