Routes that cross a previous alignment without an interchange

Which highways have been re-aligned onto a freeway, cross over/under a previous routing (excluding TEMP routings), but without an interchange between the two roads?

US 6 has a previous alignment along Hohman Ave in Munster, crossing under I-80/94 (carrying the current alignment of US 6) with no interchange.

US 31 has a previous alignment along Madison Ave in Indianapolis, crossing over I-465/74, which currently carries US 31.

Another one that technically fits is that the current alignment of US 150 along I-64 in Louisville passes over the K&I bridge, that used to carry US 150 but no longer carries vehicles at all so obviously there is no interchange.
Indiana: counties 100%, highways 100%
Illinois: counties 100%, highways 61%
Michigan: counties 100%, highways 56%
Wisconsin: counties 86%, highways 23%

There's at least one instance of this (possibly more) on US 15 in northern Pennsylvania.

The two I know about for Iowa:
1. Now that US 6 has been rerouted onto I-80 in Council Bluffs, it now passes over McPherson Avenue, the old pre-interstate alignment of US 6.
2. Lewis Blvd. in Sioux City passes under US 75. It's too close to the I-29/I-129/US 20/US 75 interchange to get an exit.

Yeah, that's the first road that came to mind when I clicked this thread.
I believe this could be the one you're referring to. Further south, there's this single ramp, with no northbound access to the old alignment until PA 35.

There's a number of these in Western Maryland, where US 40 (realigned onto an overlap with I-68) crosses over and under the National Pike now signed as Alt US 40/Scenic US 40/MD 144 depending on the location.  Here's one of the bigger examples.
"Now, what if da Bearss were to enter the Indianapolis 5-hunnert?"
"How would they compete?"
"Let's say they rode together in a big buss."

At least once for WV 55 (US 48/Corridor H) between Moorefield and Wardensville.
Government would be tolerable if not for politicians and bureaucrats.

Old US 51 crosses I-57/US 51 near Mounds, IL just north of Cairo

The US 31 freeway also crosses over its old alignment just south of South Bend with no interchange.

US 30 expressway crosses over (or under) Lincoln Hwy (aka Old US 30) a couple of times between the Ohio/Indiana state line and Delphos, Ohio, and again east of Wooster.

US 220 does south of Altoona, PA where the current route does cross over in a rural part of the area over old US 220 (now a local named road) without an interchange.
Every day is a winding road, you just got to get used to it.

US 65 & US 24 cross over Old US 24 in Carrollton, MO.

Old IL Rt 53 (before it was I-355) crosses over IL Route 53 Rohlwing Rd just south of I-290 in Addison with no interchange. I believe this is the rare instance of a route being on a freeway or expressway and moved off of it later.

I wonder... how many times does MA-128 cross over its pre-freeway alignment without an exit?

There's also no specific Robert St. exit off I-94 while it's multiplexed with US-52.

In the western end of Wayzata, MN, Wayzata Blvd. crosses the US-12 freeway without an interchange.

MN-77 (Cedar Ave) also crosses Nicols Rd (formerly Cedar Ave) on the south banks of the Minnesota River without an exit.

I-5 crosses over the former US 99W alignment south of Williams without a interchange; the nearest ones on other crossing roads are about a half-mile south and a mile north.  North of there, north of Maxwell, it crosses that same alignment, but does provide SB off/NB on diamond ramps but none for opposite movements (there is a parallel RR track to old US 99W there).
