It can be either or.  MUTCD R6-1 is the one you posted, the other version (in 1995hoo's posted photo) is MUTCD R6-2.
Some states (Massachusetts among them) have adopted a design policy whereby the R6-1 sign is used where the direction of the one way street is coming into an intersection, but the R6-2 sign is used where the direction of the one way street is going away from the intersection.
