No, it should have its own 2di.  Perhaps an eastern I-82?  It could be designated as-is.

Probably. Though I can think of a few 2 lane roads that have 4 lane divided segments almost that short.
Wait, it's all Ohio? Always has been.

Some mall entrance roads are probably shorter. I measured ~260 feet for one of the entrances to the Mall of Louisiana.
