This phrase is still a very commonly appearance in Nevada. Sign code RNV1-3 in NDOT's Highway Sign Supplement includes the text "On Left Arrow Only" underneath a pair of left/U and left arrows. This sign is commonly installed at double-left turns, even at several signals not on NDOT-maintained roads.

Signs like the Nevada example (double left arrow with "On Left Arrow Only" and supplemental "No U Turn" plaque below) are no longer installed at double left turn lanes. The current practice favors the RNV1-1 standard sign (double left arrow with "ONLY" text) next to a symbolic "No U Turn" sign.




One issue I have with the "On Left Arrow Only" text is that Nevada uses all-arrow left turn signal displays at most of it's signalized left turns (and all new left turn signal installations for the past 20+ years). It would make more sense if the standard sign were changed to say "On Green Arrow Only", since the turn signal also includes red and yellow left arrows. ("On Left Arrow Only" seems to be a holdover from early days of left arrows, when it was more common to see a display with a green arrow and red/yellow balls.)

I feel that same way too in regards to your issue.  With "On Left Arrow Only", that was when 3M Programmable Visibility Signals were utilized for left turn signals.

Here is what signs should really say (with a little Photoshop involved):
