For Colorado -- Without looking at any reference material:

US Highway:  Used to be US-666, but I believe it's "replacement", US-491, still keeps it at the highest (??)

"Private" (Tolled) Highway:  E-470 -- even has it's own styled shield.

State Highway:  Used to be CO-789 (although still listed on some mappery), I want to guess C(O)-470 (??)

Alaska: AK 98. There are higher-numbered Forest Service roads, but I don't know their signage status.

Hawaii: Interstate H-201, HI 8930. For the county routes probably CR 581 on Kauai, with perhaps one remnant CR 803 sign on one of Oahu's no-longer-officially-numbered county routes.

Going into Canada's Arctic territories:

Yukon: YT 37

Northwest Territories: NT 8

Nunavut: no highways there

Also, Quebec's Autoroutes top out at A-973. I don't know about its other provincial routes, which go at least into the high 300s.

AAA jumped the gun and marked it as I-576 on a Pittsburgh city map some years ago, but it is most definitely not an Interstate yet. It's PA Turnpike 576.

No reference material here, either. It's more fun that way. I-680 around Youngstown for the interstate and OH-835 in Dayton.
