That's a common misconception.  It wasn't renumbered.  It's still US-666.  Those US-491 shields are technically errors.  The actual US-491 runs immediately to the west of I-15, between the southern terminus of US-91 and just north of Willard Bay State Park.  Due to sign theft along US-666 and the low AADT on actual US-491, UDOT decided to sign the former with shields they removed from the latter (and unused extras that were stored in the sign shop).
