
Alabama State Route 5 travels 197.78 miles from U.S. 43 at Thomasville to U.S. 278/SR 13-74 in the small town of Natural Bridge. SR 5 overlaps with U.S. 11 northeast from Woodstock to U.S. 78 in Birmingham, and with U.S. 78 northwest to Interstate 22 between Adamsville and Graysville. SR 5 follows the former route of U.S. 78 northwest from I-22 to Dora and Jasper shifting northward to Nauvoo and Lynn.



County Road 27 branches south from SR 5 and returns to the state route within the next five miles. CR 27 parallels SR 5 throughout Bibb County, with at least four crossovers.

SR 5 south at U.S. 82. SR 219 Truck returns to SR 219 east from SR 5 via U.S. 82. SR 25 Truck joins SR 5 south from U.S. 82 west. The western section of the U.S. 82 bypass around Centreville was completed in early 2002.

Continuing beyond the parclo interchange with U.S. 82, SR 5 meets CR 58 where a roundabout was constructed in 2018/19. Branching southeast from U.S. 82 into Centreville, CR 58 is the original alignment of the US highway. Eventually CR 58 transitions into SR 58.

SR 5 and U.S. 80 converge at a diamond interchange 28 miles east of Demopolis. U.S. 80 continues east from the exchange 24 miles to Selma and 69 miles to the capital city of Montgomery.

County Road 29 stems south from SR 5 at Alberta to Gee's Bend Ferry crossing the Alabama River to SR 28 north of Camden.
