SR 684 extends west from South Bradenton to Bradenton Beach on Anna Maria Island. The signed portion of SR 684 lines Cortez Road between SR 789 (Gulf Drive) and the intersection joining U.S. 41 (44th Avenue W) with U.S. 41 Business (14th Street SW) north to Downtown Bradenton. The easternmost section of the 8.57 mile long route is the hidden state road for U.S. 41 (44th Avenue W) to 1st Avenue W.
SR 684 crosses Anna Maria Sound and the Gulf Intracoastal Waterway across the Cortez Bridge. Built in 1956, the bascule bridge is substandard with no shoulders and a narrow separation between the travel lanes and sidewalk. Having exceeded its 50-year service life, continued deterioration from corrosive salt water will further add costs to maintain the span. $6.7 million in design work is underway for a mid or high rise replacement span.

The Cortez Bridge spans Anna Maria Sound between Bradenton Beach and the community of Cortez.
