Beginning at the Alabama state line west of Jonesville, SR 166 heads 63.25 miles east through Carollton en route to Atlanta. SR 154 accompanies the highway east from Douglas County onto Campbellton Road to East Point. The two routes meet Interstate 285 where they upgrade to Langford Parkway, a four lane urban freeway. Futher east, SR 154 splits with SR 166 to join U.S. 29/SR 14, while SR 166 remains along Langford Parkway to I-75/85 (Downtown Connector) and a conclusion at Lakewood Avenue in south Atlanta.

Unconstructed Interstate 420

Named Lakewood Freeway until July 1995, Georgia State Route 166 was once part the proposed alignment for Interstate 420 approved by the American Association of State Highway and Transportation Officials (AASHTO) on June 17, 1975. Meant to provide traffic relief to Interstate 20 through central Atlanta, I-420 was first proposed as part of an urban loop beginning at Douglasville and ending at Gresham Park in Dekalb County.

The western leg of the eventual Lakewood Freeway was dropped early during the planning phase, so that the loop would start at the Perimeter Highway (I-285) instead of I-20 in Douglas County. With community opposition arising in neighborhoods east of the Downtown Connector and in adjacent Dekalb County, I-420 was never completed. Although still included in the 1988 Metro Atlanta Planning Map, I-420 was decommissioned by the mid 1980s, with the Lakewood Freeway canceled beyond the half diamond interchange with Lakewood Avenue, just east of I-75/85.
