Measuring 8.31 miles in length within northern Baltimore County, Maryland Route 138 meanders east from MD 45 at Hereford along Shepperd Road to MD 562 (Troyer Road) at Shepperd. Overtaking Troyer Road from MD 562, MD 138 arcs northward to Troyer and the Hartford County line. The route extends another 0.93 miles east along Troyer Road to MD 23 (Norrisville Road) at Blackhorse.

MD 138 turns northward from Shepperd Road onto Troyer Road at MD 562 and Shepperd. MD 562 spurs 1.39 miles south along Troyer Road to Houcks and along Old York Road to Markoe Road.
