MD 299 branches southward from U.S. 301 (Blue Star Memorial Highway) in southern Cecil County to the unincorporated community of Massey. Massey Sassafras Road leads the state route 0.49 miles south across the Sassafras River into Kent County. The 5.18 mile course in Kent County follows Galena Sassafras Road southwest to MD 290, Massey Road south to MD 313 (Massey Galena Road) and MD 330 (Massey Delaware Line Road) at Massey.

MD 299 previously continued north of U.S. 301 along Sassafras Road to Cecilton Warwick Road (MD 282), and east through Warwick to form a multi state route with SR 299 to Middletown and Odessa, Delaware. With the extension of U.S. 301 northward along the Eastern Shore, MD 299 was dropped through Warwick and partially replaced with an extension of MD 282 east to the state line. SR 299 remains posted in neighboring New Castle County.

The Maryland Department of Transportation (MDOT) recorded 921 vehicles per day (vpd) along Massey Road in 2016 and 811 vpd along MD 299 through Sassafras.
