The main ones that comes to mind in New York are I-86 EB - I-390 NB and I-390 SB - I-86 WB. Surface roads are faster for any move between the I-390 corridor and the west aside from origins/destinations right at I-390 Exit 1. NY 36 is the preferred route between I-390 and Hornell, while NY 21 is faster to Cohocton or Wayland.

You do see vehicles using the loop ramps for M-5 to/from the I-696 to I-275 ramp, but not often.

There actually is more traffic on those ramps than you might think because of vehicles going between all directions of I-96/I-275/I-696 and the M-5/Grand River Avenue/Ten Mile Road interchange; that short stretch of M-5 is a well-used spur to connect all those freeways with the surface road grid.

I think that ramp movement gets a little more usage than expected only because it’s the required movement for Toll Road traffic in either direction to access Indiana 51 south. Otherwise, there really isn’t a reason (outside of construction or major accidents shuttering the I-65 ramps nearby) to make that movement - a point rather subtly made in the exit signage for eastbound travelers (no mention of I-80 west or Des Moines on any of the signs).

Speaking of which, the connection from I-80/94 west to I-65 north doesn’t see much action (even less so with the likelihood of Gary Railcats Baseball being cancelled) outside of usage as a detour or a quick connect to 15th Avenue. The same can be said for the ramp from I-65 south to I-80/94 east.
"Slower Traffic Keep Right" means just that.

Since the I-57/294 interchange opened I am sure the I-294SB to I-80WB ramp is hardly used as is the I-80EB to I-294NB ramp.

I-75 NB to I-675 SB on the northern end. I use that ramp often enough though to make the loop back to Saginaw but not very many other people do. If you wanted to go to I-675 most people going north would use exit 150 and most people going south would use exit 155.

I-75 NB to I-475 SB also on the northern end. I-475 doesn't have a ramp to connect SB I-475 to NB I-75 and I-75 doesn't have a ramp to connect SB I-75 to NB I-475 on the southern end. But the northern end doesn't get used much.

Out west, I'll nominate I-86 WB to I-84 EB.  Not many towns from that junction to SLC on I-84.  It would almost always be quicker to take I-15 south to any areas of population.

I was on the SB I-75 to NB I-675 ramp about an hour ago lol. When you are coming from M-81 to downtown Saginaw it's quicker to go that way rather than continuing on M-81 to SB M-13.

Those aren't real high numbers for I-475 either but that's a rather dead part of Flint. SB I-475 is also useful for going from SB I-75 to EB I-69.

That might be the least-used Interstate-Interstate movement in PA. There is no reasonable path where that is the fastest route. Maybe if a trucker is doing everything possible to maximize freeway use?

Very good example! The NYS Traffic Data Viewer shows:

I-390 SB to I-86 WB: 172
I-86 EB to I-390 NB: 111

I think that's the clear winner so far for an interstate-interstate connection.
My bet is that even I-86 to I-90 in PA is going to be higher than that just because it's a more populated area and a more heavily-used travel corridor.

The last one has no purpose. It saves 3 minutes to take PA 89. Maybe it gets marginal traffic to North east.

The best (and possibly only) example I can think of from MD is I-68 EB - I-70 WB and I-70 EB - I-68 WB.  The latest data from MDOT SHA shows AADT of 222 on 68 EB to 70 WB, and 182 on 70 EB to 68 WB.

Not sure what stores are in this part of PA, but it's probably quicker for most in western Maryland just to go north into Pennsylvania.

What's the AADT on the ramps at exit 2 and 2A on NB I-675? The first one is 5th and 6th Avenues and it leads to a pretty dead part of town for the most part and use to be the downtown exit before the Warren Avenue exit was built about 10 years ago. Exits 3 and 6 will be much higher and exit 1 probably a little busier but not much.

can't find traffic data on individual ramps in South Carolina but a candidate for a low total would be the I-26 EB/I-95 NB movements and reverse.

With 4-laned US 301 connecting the two north of the 26/95 interchange there is almost no need to make these movements.

I feel like I-57 south to I-55 north could be used by people in extreme southern Illinois going to St. Louis.
It would be about 10 minutes slower vs. IL-3 and IL-146 into Cape Girardeau to I-55.

I'm not saying any of those connections aren't used, but they have generally low traffic volumes compared to the other connections based on the way the highways approach each other. For instance, all the other connections at the I-55 / I-57 / US-60 interchange would have higher volumes than that particular movement. I didn't confirm any of these with actual AADT counts, but this is just largely assumptions based on the highway locations and population centers.

Going from immediately west of the US 60 intersection in Wickliffe to I-55 north of Exit 99, Google gives 1 hour 2 minutes (63.6 miles) via US 60, I-57, and I-55.  Via IL 3 it gives 55 minutes, 46.9 miles.  The interstate option might be better than dealing with slower drivers, farm equipment, and the possible revenue patrols on the Illinois side.
