I-75 to I-75 switching from the Chrysler Freeway to the Fisher Freeway in downtown Detroit.

That's essentially a pull-through.  There is no exit number or TO banner.  Same in the other direction.

OP here.  I've seen these other examples.  Unless I missed it, I didn't see any like my US160 example where it directs you to leave the highway you're already on in order to get to that same highway.  Did I miss any?  It seems like many of the others just have a superfluous "To" sign.

How about this on NY 36?

Then there's the I-90 with the Thruway.  The western end isn't obvious, but the MassPike is I-90 in its entirety.

Here's an example from Damascus MD that I've always found interesting:

On both directions of MD 27 entering downtown, there are 'TO' signs telling you to turn off 27, take a small shortcut along Lewis Drive & High Corner Street that bypasses the 27/108 intersection, and then rejoin 27 on the other side of downtown.  (In both directions, the signs directing you to 108 is the way 27 technically goes.)

Doesn't this qualify?
"Now, what if da Bearss were to enter the Indianapolis 5-hunnert?"
"How would they compete?"
"Let's say they rode together in a big buss."
"Is Ditka driving?"
"Of course!"
"Then I like da Bear buss."
"DA BEARSSS BUSSSS"

And that's not the only one for NY 36 in Dansville.

NY 36 is straight ahead, and the hospital is on NY 36, so you turn onto Jefferson St. to get from 36 to 36. (Everybody locally uses it as a NY 36 in-village bypass.)

Gotta say I did not appreciate those signs when trying to clinch MD 27
