Freeway off-ramps that exit off of on-ramps

As a resident of Kansas City, MO, I've always found it interesting to see that on the northbound on-ramp to US 71 (Bruce R. Watkins Drive) from Cleaver II Boulevard, there's a small exit ramp for 45th Street and Garfield Avenue which branches off the on-ramp! And there is more: on that same on-ramp, there is yet another off-ramp to 43rd Street! (Street View link: https://maps.app.goo.gl/uMijMXMARdGpTFnf6?g_st=ic)

That rather unusual setup made me wonder: have you seen any other similar "off-ramp branching off an on-ramp" configurations in any other cities throughout the country? If so, where might they be?

Not sure if this is exactly what you're looking for, but what comes to mind is the ramps to Mahoning Ave that branch off from the interchange where I-76 and I-80 bump west of Youngstown, OH. In particular, the ramp that provides access from the Turnpike to Mahoning Ave essentially exits from the on-ramp to I-80 WB.

It gets interesting, though, because there are a ton of C/D road examples like this, where the roadway is both an exit ramp and an entrance ramp for a short stretch by design. For example, this roadway adjacent to I-490 WB is both the entrance from NY 31F and the exit to Commercial St. Unless I'm missing something in the OP example, this is seemingly very similar, in which case it is a relatively common configuration.

There are a few examples I can think of, many in MA and one former example in Maine:

Massachusetts:
Quincy: Independence Ave onramp to I-93, US 1 and MA 3 North has access to the Burgin Pkwy NB (though its ramp connects to MA 3's Exit 42 to the same destination)
Medford: to Fountain St from the MA 60 to I-93 NB onramp, and both sets of I-93 Northbound and Southbound ramps from the Roosevelt Circle to Fellsway W / MA 28
Somerville: I-93 SB onramp from the Fellsway / MA 38 (Mystic St)

The Fellsway W one may be questionable because to stay on Fellsway W in either direction, it first has to merge with I-93 offramp traffic, go thru the Roosevelt Cir, then use the I-93 onramps so the Fellsway W can continue.

The Quincy and Somerville ones are questionable since they connect to exits already on the mainline (braided ramps).

Maine (former):
At ME 222 in Bangor, you used to be able to exit to Ohio Ave from the I-95/ME 15 NB onramp. I think this ramp was removed in either 2014 or 2015.

Sorta similar, but I-93/Route 3 in Quincy and Route 3 in Braintree both have offramps that lead to a rotary that runs under the highway.

If I'm understanding the question, the West Falls Church Metro stop in Virginia seems to qualify. There's an off-ramp to the Metro that splits off the on-ramp from eastbound VA-7 to eastbound I-66. A few years ago they added a short ramp to connect the off-ramp from eastbound I-66 to VA-7 with the on-ramp in question to allow eastbound traffic to exit from I-66 to the Metro stop without having to go through the streets.

I-4 to I-275 in Tampa has an exit to Downtown Tampa.

I-4 WB to Epcot Center Drive near Disney has an exit ramp to the parking garages at Disney Springs.

There is an on-ramp to US 75 (from Renner Road) underneath the Bush Turnpike interchange that has an off-ramp to 15th Street in Plano (north of Dallas).  This was put in place after problems in recent years with traffic flow issues between those entering US 75 NB from EB/WB Bush Turnpike, and those wanting to exit to 15th Street from NB US 75.

The on- then off- of this part of US 75 acts a bit like a service road bypass of the signaled intersection at Plano Parkway, also.

Las Vegas' I-15 Project Neon introduced something like this. The I-15 NB on-ramp from Charleston Blvd has slip ramps to Grand Central Pkwy and to Alta Dr/Bonneville Ave (Street View). The only difference here is that those additional destinations are signed in addition to Charleston Blvd from the I-15 northbound exit (Street View), with this access being added to prevent the need for drivers to turn east on Charleston and immediately dart to left turn lanes for Grand Central.

When you are going from the Lodge Freeway SB to the Fisher Freeway SB you have the Trumbull Avenue exit that exits off the on ramp to the Fisher.

In Perth Amboy, NJ the on ramp to Route 440 NB from Route 184 EB has an exit to Amboy Ave.

The ramp from the I-695 Baltimore Beltway to I-95 SB has a lone ramp leading to Sulphur Spring Rd.  This is where they intersect on the SW side of town.

And at the US-50 (unsigned I-595)/US-301/SR-3 interchange in Bowie, MD, two of the movements have direct ramps to Medford Blvd.

In Oakland County MI the recently reconfigured ramps from I-696 to northbound I-75 join together then have an exit to 11 Mile Rd. before joining I-75. Northbound I-75 has its own separate exit ramp to 11 Mile that crosses under the I-696 ramp. The major reason for the reconstruction was to eliminate the bottleneck on I-75 that was caused by the I-696 entrance ramp lane becoming an exit only lane for 11 Mile.

In Dayton OH, there is a stack interchange with I-75 and US 35. The ramps from I-75 to US 35 westbound have an exit for Germantown Pike shortly before they merge into the mainline.
