Looks like a little ramp to me.  Doesn't look like a big issue.

On PA 8 SB at Debence Rd/Polk Cut Off south of Franklin, PA (https://www.facebook.com/photo/?fbid=10220990037301147&set=a.10220990124743333)  (Signs are not unique, but the combination of the intersection warning sign with a green sign with the road name is abnormal.)

You used to see this combo a lot in Nevada. Then the 2009 MUTCD specified street name guidance placards for intersection warning signs should be black on yellow, and most instances have been changed out when signs were replaced. 

With the simultaneous adoption of mixed case lettering for street name signage, the black on yellow vesting are far less readable to me.

I think it's interesting that Caltrans didn't use a text shield here, but tacked on an actual green CA 1 shield.

Yes I'm aware this is one of many as I am in have plenty of other photographs of other CA state routes as well.  However I'm not going to post them all to make a simple point that I find this practice fascinating. Being the shields are green anyway I would assume use a text shield, but no the lighter green of the shield on a darker green assembly is fine by them in many instances.

The VMS beneath the pull through is pretty unusual, but a necessity. There are indeed two exits for Hoboken on both sides to avoid weaving here.

However more oddities though like the size of the pull through as well as the guide sign for Hoboken's right exit being black on yellow and the redundancy between the pull through and VMS information ( the Turnpike and right Hoboken ramps).  Plus the arrow on this pull though is not needed as the green arrows beneath are doing the job of lane control anyway.

Then the addition of Weehawkin to the right exit as for many decades Weehawken was only signed for the next following exit for Kennedy Boulevard East as that's the exit for its business district.  In addition the second following exit at Park Avenue is now signed also for Weehawken.

So the engineer who signed this is being way too literal and not following procedures.


Edit:  The top link sign has been edited to eliminate  the yellow guide sign for the right ramp and VMS to guide motorists to the fact two ramps depart from both outside lanes.

It seems odd that NYSDOT don't erect a large green guide at the interchange here instead of the shields that they use.

They're not particularly common around here, but that is a standard MUTCD sign (W10-11B).

I was about to say I've seen them a lot in Georgia, I think Illinois likes to use them too?
