Interstate 165 extends 70.185 miles along the William H. Natcher Expressway between I-65 near Bowling Green and U.S. 60/231 in Owensboro.

Beginning at a cloverleaf interchange along I-65 in Warren County, I-165 proceeds northwest to line the southern and western edges of Bowling Green en route to Morgantown. The Natcher Expressway continues northwest from the Butler County seat, crossing the Green River ahead of entering Ohio County. I-165 meets the Western Kentucky Parkway (future I-369) at an exchange midway between Bowling Green and Owensboro, southeast of Beaver Dam. KY 69 joins the freeway with Hartford before continuing along a 22 mile exit less stretch to its conclusion at a trumpet interchange (Exit 72) with the U.S. 60/231 freeway bypass in southeast Owensboro.

Approved in Fall 2017 by the American Association of State Highways and Transportation Officials (AASHTO), signs for I-165 were installed by Summer 2019 for 70 miles of the Natcher Expressway. The southernmost 2.079 miles between I-65 and U.S. 231 (Scottsville Road) was signed as KY 9007, based on the internal designation (WN-9007) used by the Kentucky Transportation Cabinet (KYTC). House Joint Resolution 146 under the Kentucky General Assembly also renamed the Natcher Parkway as the Natcher Expressway upon its designation as an Interstate highway.1
