NY 245 angles northeast from NY 21 in the village of Naples to Geneva within the Finger Lakes Region. The 28.53 mile long route travels east of Canandaigua Lake through the towns of Italy, Middlesex, Potter and Gorham. The northern 3.05 miles overlap with NY 14A from the town of Seneca to U.S. 20/NY 5 just west of the Geneva city limits.


NY 245 begins alongside NY 14A south from U.S. 20/NY 5 at Lenox Park west of Geneva. The two routes overlap 3.05 miles southward into the town of Seneca.
