Beyond the commercialized stretch through south Adrian, U.S. 223 resumes along a rural course southeast through Palmyra Township to the village of Blissfield. Entering Monroe County from Riga Township, U.S. 223 meets Memorial Highway south to the unincorporated community of Ottawa Lake. Memorial Highway extends 5.74 miles to the Ohio state line along the former route of U.S. 223.

U.S. 223 Business serves the city of Adrian while U.S. 223 bypasses the city center to the south. U.S 223 Business branches east along Maumee Street 1.84 miles to Downtown Adrian. The bannered route shifts onto Church Street for 0.26 miles to M-52 at S Main Street. U.S. 223 Business returns to U.S. 223 along a 1.65 mile overlap with M-52 (S Main Street).

Part of a $40 million resurfacing and shoulder widening project, construction from April 10, 2023 to October 13, 2023 replaced the intersection joining U.S. 127 and the north end of U.S. 223 with a roundabout. The project improved 14 miles of roadway along U.S. 127 from U.S. 12 south to Junction Road at the village of Addison.1
