There's quite a few HOV left exits, but I don't feel up to listing them all.

Left exits are quite rare in the Bay Area, but they're used on the Bay Bridge, and on I-280 SB in its Northern junction with US-101.  Also, there are a few HOV left exits.
Take the road less traveled.

It would be interesting to differentiate between left exits at system (freeway-to-freeway) and service (freeway to surface road) interchanges.  In Wichita, there are no left exits at service interchanges, with the only movements that could plausibly be signed as left exits being at splits within system interchanges (I-135/I-235 "North Junction" and I-135/I-235 near the South Wichita interchange on the Kansas Turnpike).

Freeway splits are an ambiguous case because it is essentially an arbitrary decision to sign them as left exits and other conditions have to be met at an individual split before it can be considered to have the operational disadvantages ordinarily associated with left exits.  For example, in a TOTSO situation at a standard wye interchange, the mainline route might take the right-hand branch of a split, which would require that the left-hand branch be treated as a left exit under some state exit numbering policies, but if traffic on the left-hand branch merged onto a new freeway from the right, the movement would not have any of the operational disadvantages of a left exit or left merge.

I-405 North in Portland is practically an entire left exit in itself! From I-5 North to I-405 North is a left exit, not to mention the very first exit (SW Naito Pkwy) is a left exit. Continuing on I-405 North, the next left is US 30 West, which is then followed by the US 30 East exit (technically just a movement to I-5 South). Other examples of a left exit in the Portland metro area:
I-5 South -> I-405 North
I-5 South -> Mt. Hood Freeway (the ghost ramp still exists)
Outside of the metro area, I-84 at Multnomah Falls (both directions)

A few of these in the LA County area:
I-5 North at Firestone Blvd
I-5 North -> I-710 North
I-710 South -> I-5 South
US 101 South -> I-10 East
Of course, there are the very numerous HOV left exits in SoCal...

In Fairfield County, CT (total of 5):

All are freeway to freeway. One is a major split and thus only sort of counts.

In Westchester County, NY (total of 18):

The DOT finished it's study of the I-84/CT-8 interchange, if you look at Concept 7, it has one lone left on-ramp from I-84 EB to CT-8 NB.  Ugh!

Hampton Roads:

- I-64 westbound at VA 143 (Williamsburg Business Route)

I'm proud to live in a state which does not have a single left hand exit or entrance on a mainline interstate.  The only left exits are those from where an interstate ends and splits (such as I-93's terminus, splitting into ramps to I-91 North and South or I-189's terminus, but both ramps enter on the proper side of the interstate).

My closest left exit would be the one on I-89NB just outside of Concord NH, or Exit 27 on I-91SB near Greenfield MA.

Birmingham, AL's I-20/59 - I-65 interchange has the following left-lane exits:  I-65 NB to I-20/59 WB/SB,  I-20/59 WB/SB to I-65 SB, I-20/59 EB/NB to I-65 NB , and I-65 SB to I-20/59 EB/NB are all left exits, with corresponding left entrances.
