Not quite.  I have a photo of a NJ 413 shield from November 2008, east of the toll plaza.  However, most other signs relating to NJ 413 mention PA 413, as it is the much more prominent route.

Also, a photo from April 2010 that also shows a NJ 413 shield...

VT 26 is actually well signed as such from US 5 although it immediately becomes NH 26 (and I do mean immediately).  In contrast, NY 120A has a single CT 120A shield (maybe there are two, but that's about it) because it straddles the border, but there is no such thing as CT 120A.

CT-120A even has NYSDOT tenth mile markers. I have another shot of an old  NJ-413 West shield from the first circle, its long gone though. NJDOT doesn't even maintain NJ-413, its a touring route, hence the crappy signing.

If you always take the same road, you will never see anything new.

I know another one, PA's US-206 which i think is maybe just the approach to the US-206 bridge over the delaware. only 1/2 a mile is in PA

I just saw that sign today, it is looking the same as it did in 2008, crappy.

There are still signs back in Milford on US 6/209 where they split.  That's either 1 or 2 miles from the approach.

I'm not sure if it's what you're looking for, but NJ-495 is a quirky little route that's not super overtly signed. I think there's maybe one green sign on the side of the road after the toll plaza with the turnpike, but I can't think of any others... http://en.wikipedia.org/wiki/New_Jersey_Route_495 Are there any reassurance markers on it at all after the toll plaza?

Yes, there is. Even has trailblazers on local streets near the onramps. The shields are more like rounded rectangles then ovals though.
