NY 15 overlaps with U.S. 20 west from Livonia to Lakeville on the north side of Conesus Lake. The state route resumes a northward course from there, paralleling Interstate 390 north through the town of Avon, town of Rush and Henrietta through the south Rochester suburbs. NY 15A returns to NY 15 (Mt. Hope Avenue) in south Rochester. Mt. Hope Avenue leads the route north to Byron Street south of I-490. NY 15 extends 0.30 miles north along Clinton Avenue into Downtown Rochester.

Truncated south from Rochester to Painted Post in 1974, U.S. 15 was decommissioned in part because the Southern Tier Expressway (NY 17) and construction of Interstate 390. Old U.S. 15 was renumbered as NY 415 where it paralleled the freeway system between Painted Post and Cohocton. NY 15 was signed alongside by NY 17 and I-390 north to Wayland for continuity purposes. This eventually changed with NY 15 dropped south of Wayland in 2009, though several signs remained posted as of 2017.
