South Carolina Highway 34 travels 185.93 miles across the Palmetto State from Greenwood to Dillon. Beginning from U.S. 25-178 Business (Main Street S) in south Greenwood, S.C. 34 follows Ninety Six Highway east to Main Street across the town of Ninety Six. Passing south of Lake Greenwood across the Saluda River, S.C. 34 enters Newberry County.

S.C. 34 continues east to the town of Silverstreet, where a 4.53 mile long overlap with S.C. 121 begins. Advancing to the Newberry County seat of Newberry, S.C. 121 separates around the north side of the city while S.C. 34 runs east along Dixie Drive to U.S. 76 (Wilson Road). Boundary Street east to S.C. 395 (Nance Street) in Downtown Newberry, and Main Street east to Winnsboro Road were formerly part of S.C. 34 Business.

U.S. 76/S.C. 34 overlap for 0.90 miles to Winnsboro Road in Newberry. S.C. 34 turns east there to meet Interstate 26 at Exit 74 and U.S. 176 at Keitts Crossroads. Spanning the Broad River, S.C. 34 enters Fairfield County en route to U.S. 321 outside Winnsboro. S.C. 34 combines with U.S. 321 south 3.64 miles to bypass the Fairfield County seat.

Continuing east from Winnsboro, S.C. 34 converges with Interstate 77 just west of the town of Ridgeway. Within Ridgeway, S.C. 34 joins U.S. 21 (Palmer Street) for 0.22 miles between Church Street and Broad Street. S.C. 34 bee lines east from there to Lugoff in Kershaw County.

S.C. 34 overlaps with U.S. 1/601 north from Lugoff into the city of Camden. U.S. 601 turns north alongside U.S. 521 (Broad Street) while the 6.11 mile long concurrency with U.S. 1 (Dekalb Street) extends east to Bishopville Highway. S.C. 34 parallels Interstate 20 to the north from Camden to the Lee County seat of Bishopville.

U.S. 15 and S.C. 34 combine for 11.35 miles from S.C. 341 (Church Street) in Downtown Bishopville to S.C. 403 at Lees Crossroads in Darlington County. U.S. 15 turns north to Hartsville while S.C. 34 proceeds east to Darlington. Traversing wet land areas and the Pee Dee River, S.C. 34 continues east to Brownsville in Marlboro County.

The eastern most extent of S.C. 34 follows a winding course northeast to Bingham in Dillon County and east to Interstate 95 at Exit 190. The state highway concludes along Main Street in the city of Dillon at the south split of U.S. 301/501 and S.C. 9.
