U.S. 220 bypasses Martinsville to the west along a freeway bypass while U.S. 220 Business runs 10.78 miles to the east through the independent city and Collinsville. The business route extends Greensboro Road north from U.S. 58/220 (William F. Stone Highway) to Martinsville Speedway and U.S. 58 Business (A.L. Philpott Highway). U.S. 58-220 Business combine north across the Smith River along Memorial Boulevard into south Martinsville.

U.S. 58 Business combines with SR 57 northeast along Starling Avenue around Downtown Martinsville. U.S. 220 Business/SR 57 overlap to the northwest past the city center to Fayette Street. Beyond the city line, U.S. 220 Business continues through Henry County along Virginia Avenue to Collinsville and U.S. 220/SR 57 at the north end of the Martinsville bypass.

The Martinsville bypass opened to traffic north from U.S. 58 by Grassy Creek to SR 57 at Bassett Forks by July 1, 1976. The American Association of State Highway and Transportation Officials (AASHTO) approved both a 9.99 mile long relocation of U.S. 220 west alongside U.S. 58 from Greensboro Road to the Martinsville bypass and north along the freeway past Fieldale, and the establishment of U.S. 220 Business along 7.36 miles of existing U.S. 220 through Martinsville and Collinsville on July 13, 1976.

The remainder of the Martinsville bypass 3.62 miles south from U.S. 58 near Grassy Creek to U.S. 220 along Greensboro Road was completed by December 1, 1977. A subsequent application to AASHTO proposed extending U.S. 220 Business south 3.14 miles along Greensboro Road from U.S. 58, eliminating 2.25 mile long U.S. 58/220 overlap along A.L. Philpott Highway and realigning U.S. 220 along the completed freeway. AASHTO concurred with the request on October 29, 1977.
