U.S. 220 begins in the city of Rockingham from U.S. 1 south of Downtown. Following an older bypass, U.S. 220 passes west of the business district en route to I-73/74 in northern Richmond County. Construction of I-73/74 replaced U.S. 220 north to Ellerbe.

U.S. 220 loops west from I-73/74 through the town of Ellerbe, then parallels I-73/74 more closely north to the town of Norman. Shifting east of the freeway, U.S. 220 continues to Emery, where it rejoins I-73/74 north past Candor.

The remainder of U.S. 220 north from Candor to Asheboro and Randleman overlaps with I-73/74. Interstate 73 continues along U.S. 220 north to I-85/U.S. 421 on the south side of Greensboro. U.S. 220 extends north from the Greensboro Urban Loop along a freeway to I-40 at Freeman Mill Road.

Circumventing Downtown Greensboro, U.S. 220 overlaps with I-40 and U.S. 29 east to O'Henry Boulevard. U.S. 29/220 continue north along O'Henry Boulevard, a limited access expressway to U.S. 70 at Wendover Avenue. U.S. 220 turns west along Wendover Avenue to Battleground Avenue, which takes the highway northwest to I-840 (Greensboro Urban Loop) and Lake Brandt.

Advancing north through the town of Summerfield, U.S. 220 again converges with Interstate 73. Completed in 2017, the northernmost section of I-73 overlays U.S. 220 into Rockingham County beyond Stokesdale. U.S. 220 travels along an at-grade expressway from NC 68 and the end of I-73 past Madison, Mayodan and Stoneville to the Virginia state line at Price.
